wip #47: register accessor for write after open in DeviceModule if write cannot be performed in initialisation phase

change declaration of 'decrementDataFaultCounter' in 'EntityOwner' to take a bool parameter and adjust rest of the code to it
implement 'decrementDataFaultCounter' in 'ApplicationModule' according to ticket #95 -> rewrite all outputs when 'faultCounter' goes from 1 to 0

- ConfigReader is in ApplicationCore.h, but was including ApplicationCore.h itself
- Added include guards to ApplicationCore.h, although it's not stricktly needed (all included headers have one). Might speed up complilation because it makes it easier for the parser.

This is needed to add all device variables to e.g. the history module.

- Exception handling decorator and ThreadedFanOut send condition variable notificatios via the DeviceModule
- FanOut sends terminate to impl and all slaves.
Solves (some?) shutdown issues, but breaks bi-directional array test.

first implementation of device initialisation handler. Simple test passes, but needs to be extended.

  DeviceModule and ControlSystemModule now do not take the optional prefix argument in their constructors any more. Use the [] operator instead to get the submodule.

- Add ExceptionHandlingDecorator and decorate all device variables with it.

- need to catch exception by reference, else the message seems to be lost
- test if the message is correct
- test if device is really closed and opened again
- changed names of status variables in control system to match the general naming conventions
